Lakeview Grant Guidelines

1. Applicant must be a non-profit organization.
2. The grant purpose must be health related - supports community health and wellness.
3. Serves Lakeview Health’s geographic service area - core, primary or secondary.
4. Grants are restricted for the purpose for which it was requested.
5. Conditions of the grant cannot request or require an agreement that in exchange for the grant, the organization will provide Lakeview Health's member organizations with referrals.
6. Only one request per organization will be evaluated each year. Grants are generally in the range of $1,000 - $5,000. Sponsorship requests that have been received may be factored into this amount.
7. Lakeview Hospital sponsorships will be handled through Lakeview Foundation (up to individual requests of $2,500).
8. Grants will be reviewed two times a year - February and August. Requests must be received no later than the first of the month prior to the review month for consideration (January and July).
