Grant Guidelines

As of October 1, 2007, Lakeview Foundation’s board of directors has assumed oversight of the disbursement of grants on behalf of all Lakeview Health subsidiaries, including those made by Lakeview Hospital. Grants will still be paid out of Hospital funds, but all correspondence and proposals should be sent to the Foundation office.

1. Non-profit organization

2. Health related, supports community health

3. Serves Lakeview Health’s geographic service area

4. Grants are restricted for the purpose for which it was requested.

5. Conditions of the grant cannot request or require an agreement that in exchange for the grant, the organization will provide Lakeview Health with referrals.

6. Only one request per organization will be evaluated each year. Grants are generally in the range of $1,000 - $5,000.

7. Budgeting will be based on 5% of Lakeview Hospital’s net income from the previous year.

8. Annual sponsorships will be handled through Lakeview Health’s Community Relations Department. (Up to individual requests of $2,500 and total of 10% of annual budget - see policy titled: Requests to Lakeview Health for Donations).

9. Grants will be reviewed two times a year – June and September. Requests must be received no later than the fifteenth of the month prior to the review month for consideration.

Provisions of Grant Agreement

1. A specific dollar amount

2. The time frame in which the recipient must use the grant

3. The specific purpose(s) approved by the Foundation’s Board of Directors for which the grant is to be used.

4. Authorization to review grant activity when appropriate

5. A requirement that the recipient provide the Foundation with written reports according to a specific schedule; for example, monthly or annual reports.

6. A provision for the reversion of grant funds in the event that the recipient does not use all the funds by the end of the grant period.

7. A provision that if the Foundation determines that the recipient has not performed in accordance with the terms of the grant, the funds must be returned to the Foundation.
