Lakeview Grants Committee Grant Guidelines
- Non-profit organization.
- Health related, supports community health and wellness.
- Serves Lakeview Health’s geographic service area - core, primary or secondary.
- Grants are restricted for the purpose for which it was requested.
- Conditions of the grant cannot request or require an agreement that in exchange for the grant, the organization will provide Lakeview Health's member organizations with referrals.
Only one request per organization will be evaluated each year. Grants are generally in the range of $1,000 - $5,000.
Budgeting will be based on 5% of Lakeview Hospital’s net income from the previous year.
Lakeview Hospital sponsorships will be handled through Lakeview Foundation. (Up to individual requests of $2,500 and total of 10% of annual budget - see policy titled: Requests to Lakeview Health for Donations).
Grants will be reviewed two times a year – February and August. Requests must be received no later than the first of the month prior to the review month for consideration.
Provisions of Grant Agreement

A specific dollar amount.

The time frame in which the recipient must use the grant.

The specific purpose(s) approved by the Grants Committee for which the grant is to be used.

Authorization to review grant activity when appropriate.

A requirement that the recipient provide the Grants Committee with written reports. An applicant may not receive new funds unless their annual report has been received on or before the grant period deadline. In addition, please include a summary of the external communications regarding the grant/program/results (information sent to local newspapers, TV news or corporate news letters/community newsletters, etc.)

A provision for the reversion of grant funds in the event that the recipient does not use all the funds by the end of the grant period.

A provision that if the Grants Committee determines that the recipient has not performed in accordance with the terms of the grant, the funds must be returned to Lakeview Hospital.
